- Pet's complete vaccination card
- ISO microchip number (implanted here if you don't have one)
- Rabies vaccination administered after microchipping
- Rabies antibody titer report from an approved lab
- Destination country import permit or entry requirements
- Airline-approved travel crate details
- Recent deworming and ectoparasite treatment record
